Non-urgent advice: Managing Your Long-Term Conditions

At our practice, we are committed to supporting you in managing any long-term health conditions you may have. Long-term conditions are health issues that usually cannot be cured but can be controlled and managed effectively to help you live well. Examples include asthma, high blood pressure, and diabetes.

What is a Long-Term Condition Review?

Each year, we invite patients with long-term conditions to attend a review appointment with one of our experienced practice nurses. This review is an important chance to:

Talk about your overall health and well being
Discuss how your condition(s) are affecting you
Review your current treatment and medications
Address any concerns or questions you may have
Work together to create or update a personalised care plan

What to Expect at Your Review

Duration: Approximately 30 minutes
Holistic Care: We consider your physical health, emotional well being, and social factors
Personalised Support: The review is tailored to your individual needs and goals
Next Steps: We may suggest follow-up appointments with your GP or referrals to specialist services if needed

Bringing Someone With You

You are welcome to bring a family member or friend to your appointment if you would like extra support or help discussing your care.

How to Book Your Review

We usually invite patients by their birth month via text message or letter. When you receive your invitation, please call us on 01753 989800 to book your appointment.
